Bright HealthCare is no longer offering health insurance plans of any kind, including Individual and Family (ACA), Medicare Advantage, and Small Group plans.

We had stopped offering coverage of any kind on December 31, 2022 (except in Texas).

Members with policies in Texas

Members with policies in all other states

Claims: The timely filing period for ALL new claims has passed and submissions will be denied for this reason.

Appeals: The timely filing period has passed for appealing the vast majority of claim decisions. If you believe you are within timely filing guidelines, please follow the directions below for the state relevant to the policy.

Other inquiries or issues: It has been over 18 months since any active policy with Bright Health (outside of Texas) and we have very limited operations remaining as there are limited issues or inquiries that can be considered relevant at this time. Please be mindful of the few remaining resources to support requests.

Appeal Directions

Inquiries about paid claims can be submitted through the contact information in the bottom section of this web page. If an Appeal is necessary, please be sure to follow the directions for the state where the policy was valid.

All appeals must include:  
- Patient's name and Member ID  
- Provider's name
- The date(s) of the medical service(s)  
- Description of the adverse determination, including what was denied
- The reason the member disagrees with the adverse determination
- Any documentation, including medical records, to support the member’s position
- If the adverse determination is based on a contractual exclusion, the member may want to submit evidence from a medical professional indicating that there is a reasonable medical basis for the exclusion not to apply
